1. Create a Follow My Health Account

   • Visit www.followmyhealth.com to create your secure, HIPAA-compliant account.

2. Connect to the Facility of Your Choice

   • Once your account is created, search for and connect with the facility where you’d like to receive care.

3. Complete the Required Questions

   • Answer a few brief questions to help us prepare for your visit.

4. Book or Schedule an Appointment

   • Select a provider below to view and request available appointment times.

• All appointments are virtual. No in-person visits are currently available.
